Appointment Information

Appointments may be scheduled between 8:30 AM and 4:30 PM Monday through Friday by calling either office or requesting an appointment online using the information below. Click on one of the links below to schedule an appointment at that office.

When calling the office our scheduling personnel will need your address, phone number, current insurance information, and the nature of your problem. We ask that you bring along all relevant x-rays, MRI’s and test results, along with a list of your current medications if applicable.

Prescription Renewals

Please call us during office hours and before 2 PM to request a prescription or a refill (Monday-Friday).

If you need a prescription renewal that was prescribed by one of our physicians or physician assistants,
you may request a prescription renewal and your prescription will be called into the pharmacy within 48 hours.

Billing and Insurance Information

We participate with most major insurance plans; please refer to our Patient and Financial Policy.  As an added convenience, we have a dedicated telephone line for questions relating to billing and insurance matters 570-517-3761, 570-3762 and 570-3773.

Each patient is responsible for knowing the requirements of their insurance plan as well as their patient responsibility. Patients with Managed Care insurance are responsible for obtaining the required referrals and must furnish them at the time of service.

We request that current insurance cards be present at each visit.

Payment Information

For your convenience, we offer the CareCredit Plan which affords our patients another alternative payment arrangement.

Physician Assistants

Our Physician assistants have been specially trained to assist our physicians in all aspects of care. Surgery Information
Our patient care coordinators will be available for pre and post-operative education. They will be happy to discuss any aspects of your surgical procedure. If the need arises, you may be scheduled with a coordinator for an in-office visit to review information regarding your surgery. They will also be available during normal business hours for telephone consultation.
Patients scheduled for surgery will have the opportunity to review surgical information with a nurse. However, most total joint education will be completed by the hospital nursing staff where the surgical procedure will be performed.
